SimpliFiber LLC Fee Schedule

This Fee Schedule outlines the fees associated with SimpliFiber Internet, Phone, and common area Wi-Fi services (collectively, the "Services") provided by SimpliFiber LLC, located at 748 W Heritage Park Blvd #201, Layton, Utah 84041, United States ("SimpliFiber"). This Fee Schedule is incorporated into the SimpliFiber Residential Terms of Service. All fees are subject to applicable taxes and may vary by location or Service plan. SimpliFiber reserves the right to update this Fee Schedule, with changes taking effect no sooner than fourteen (14) days after posting on getsimplifiber.com, except for changes required by law or new Service features, which may take effect immediately.

Service Fees

SimpliFiber offers various Service plans, which may include Internet, Phone, or bundled packages. Monthly service fees are charged starting at the time of installation, as specified in your Service plan. Specific plan details and pricing are available at getsimplifiber.com. Any changes to monthly service fees will be communicated with at least thirty (30) days’ notice.

Installation and Construction Fees

Installation or construction fees may apply to set up the Services at your residence. These fees cover the cost of installing equipment, such as the Fiber Jack, and any necessary infrastructure work. Fees may vary based on the complexity of the installation and your location. You may be eligible to pay these fees in installments, as determined at sign-up. If you cancel or modify your Service plan, or if your Services are terminated due to non-compliance with the SimpliFiber Residential Terms of Service, you agree to pay any outstanding installation or construction fee balance. Specific fees are provided at the time of sign-up and are available at getsimplifiber.com/support.

Equipment Fees

SimpliFiber may provide or rent equipment, such as routers, switches, modems, or phone boxes, to enable use of the Services. Unless explicitly stated otherwise on your service agreement, SimpliFiber owns all equipment provided. The following fees may apply:

  • Replacement Fees: If SimpliFiber-owned equipment is lost, damaged, or not returned in working condition within sixty (60) days of Service termination (excluding purchased equipment, Fiber Jack, or inside wiring), you may be charged a replacement fee. Fees vary by equipment type and are detailed below:
    • Fiber Jack (includes fiber from jack to device): $100
    • Wi-Fi Router: $100
    • Wi-Fi 6 Router: $150
    • Wi-Fi 7 Router: $200
    • Switch: $100
    • Phone Box: $50
    • Battery Backup: $125
    • Other Accessories (e.g., power adapters): $25
  • Rental Fees: If you rent equipment from SimpliFiber, a monthly rental fee may apply, as specified in your Service plan. These fees are included in your monthly billing statement and are subject to change with notice, as outlined above.

Local Access Fees

In certain jurisdictions, SimpliFiber is required to collect local access fees as part of agreements with local governments. These fees are passed directly to the relevant jurisdiction and may vary by location. The specific amount will be included in your billing statement and is subject to change based on local regulations.

Late Payment and Non-Payment Fees

If payment for the Services is not received within seven (7) days of the due date (the "grace period"), a late payment fee of $10 will be charged. Non-payment may result in suspension or termination of your Services, as outlined in the SimpliFiber Residential Terms of Service. Additional fees may apply to reinstate Services after suspension, including:

  • Late Payment Fee: $10 if balance is not paid within the 7-day grace period.
  • Service Reinstatement Fee: Up to $25, applied at SimpliFiber’s discretion.

Other Fees

Additional fees may apply for optional services or specific actions, including but not limited to:

  • Service Change Fee: A fee of up to $50 may apply for changes to your Service plan, such as upgrading or downgrading your Internet speed or adding/removing Phone Services.
  • Technician Visit Fee: If a technician visit is required for issues not caused by SimpliFiber (e.g., customer-caused equipment damage or non-standard installation requests), a fee of up to $125 per hour may apply.

Payment Terms

All fees are due as outlined in your billing statement. You agree to pay all applicable fees, including taxes, through the payment method associated with your SimpliFiber account. Failure to pay fees may result in suspension or termination of Services, as described in the SimpliFiber Residential Terms of Service.
